目的

探讨综合干预措施联合光动力治疗在儿童尖锐湿疣(CA)中的应用效果。

方法

回顾性分析2018年1月至2024年12月于首都医科大学附属北京地坛医院皮肤性病科门诊接受光动力治疗的30例CA患儿的临床资料。在光动力常规治疗基础上,针对CA患儿及其家属实施针对性心理干预、治疗全程疼痛管控、术后创面专项皮肤处理以及健康知识宣教等一系列综合干预措施,完成6个月定期随访,统计患儿临床疗效、患儿治疗依从性和相关不良反应。

结果

入组30例患儿中男性18例、女性12例,年龄2~14岁[(6.3±2.4)岁],病程1~6个月[(4.2±1.2)个月],皮损直径0.2~1.8 cm[(1.22±0.10)cm],皮损数目1~8个[(4.43±0.8)个];病变累及外生殖器者20例(66.67%),累及肛周者8例(26.67%),2例(6.66%)患儿外生殖器与肛周病灶同时存在。经联合综合干预措施的光动力治疗后,痊愈22例(73.33%)、显效7例(23.33%)、无效1例(3.33%),临床总有效率为96.67%(29/30),6个月随访期间无复发病例。全部CA患儿完成全程治疗,治疗依从性良好,其中完全依从25例(83.33%),部分依从5例(16.67%),无完全不依从病例。治疗过程中所有患儿均未发生严重全身性不良反应,局部不良反应均为轻中度,且可在同一患儿重叠出现。共11例患儿(36.67%)至少出现1种局部不良反应,其中出现疼痛6例(20.00%)、局部红斑6例(20.00%)、水疱糜烂2例(6.67%)、瘙痒伴烧灼感1例(3.33%)、色素沉着3例(10.00%);所有不良反应经局部对症处理后均完全缓解,未影响ALA-PDT治疗进程。

结论

综合干预措施联合光动力治疗儿童CA,患儿治疗依从性良好,不良反应可有效缓解。

Objective

To explore the application effect of comprehensive intervention measures combined with photodynamic therapy on children with condyloma acuminatum (CA).

Methods

Clinical data of 30 children with CA who received aminolaevulinic acid-photodynamic therapy (ALA-PDT) at the Department of Dermatology and Venereology, Beijing Ditan Hospital, Capital Medical University from January 2018 to December 2024 were analyzed, retrospectively. Based on conventional PDT, a series of comprehensive intervention measures such as targeted psychological intervention, full-process pain management, postoperative wound skin management, and health knowledge education were implemented for CA children and their families. After the treatment was completed, regular follow-up were conducted for 6 months to collect statistics on the treatment efficacy, treatment compliance, and related adverse reactions of the children.

Results

Among the 30 children with CA, 18 cases were boys and 12 cases were girls, aged from 2 to 14 years old [(6.3±2.4) years old], the disease duration was 1 to 6 months [(4.2±1.2) months], the diameter of the lesions was 0.2 to 1.8 cm [(1.22±0.10) cm], and the number of lesions was 1 to 8 [(4.43±0.8)]; 20 cases (66.67%) had lesions involving the external genitalia, 8 cases (26.67%) involved the perianal, and 2 cases (6.66%) had external genitalia and perianal lesions. After PDT combined with comprehensive interventions, 22 cases (73.33%) were cured, 7 cases (23.33%) were markedly effective, and 1 case (3.33%) was ineffective. The total treatment effective rate was 96.67% (29/30). No case recurred during the follow-up for 6 months. All children completed the full course of treatment, and 25 cases (83.33%) were fully compliant, 5 cases (16.67%) were partially compliant. No serious systemic adverse reactions occurred during the treatment, and local adverse reactions were all mild to moderate, and may occur concomitantly on the same child. A total of 11 children (36.67%) had at least one local adverse reaction, 6 cases (20.00%) had pain, 6 cases (20.00%) had local erythema, 2 cases had blister erosion (6.67%), 1 case had itching with burning sensation (3.33%), and 3 cases had pigmentation (10.00%); All adverse reactions were completely relieved after local symptomatic treatment and did not affect the ALA-PDT treatment process.

Conclusions

After the comprehensive intervention measures combined with ALA-PDT, CA children showed good treatment compliance, and the adverse reactions can be effectively alleviated.
